WHAT ARE THE STEPS OF NURSING PROCESS?

NURSING PROCESS
1. Assessment- the first step of nursing process is the systematic, organized collection of data about the patient.Simply its "Gathering Information".
       -Need  to assess:
  •   Past History (chronic conditions,drug use,allergies,level of education,level of understanding of disease and therapy,social support,financial support,and pattern of health care)
  • Physical Assessment (weight,age,physical parameters related to the disease state or known drug effects).                            
2. Nursing Diagnosis- once data have been collected, the nurse must organize and analyze information to arrive a nursing diagnosis."Analyzing the information gathered to arrive at some conclusions."

3. Intervention - the assessment and diagnosis of the patient situation will direct specific nursing interventions." Actions undertaken to meet the patients needs such as administration, education and comfort measures."

4.Evaluation- is part of the continual process of patient care that leads to changes in assessment, diagnosis and intervention.Simply determining the effects of the interventions that were performed.

In general  the nursing process provides an effective method of handling all of the scientific and technical information as well as the unique emotional, social and physical factors that each patient brings to a given situation.Using nursing process ensures that the patient receives the best, most efficient, scientifically based holistic care.
